Include as much information in reporting the incident as you are able. If you select "Other" from one of the categories below, please provide details in the "Incident Description" text field.

Enter the date and approximate time the incident took place (the webform default is today's date)

Select the library branch and specific location in which the incident occurred. Location selections apply to both branches.

Violent Incident on Staff Member
Additional information and follow up is required for incidents of violence perpetrated on staff members (including for "near misses")
Indicate emergency services or other external parties summoned to assist in the incident; select all that apply.

Include an account of any actions taken, other than those already indicated in the "External Parties Summoned" and "Outcome" categories. Drag the bottom of the text box to expand its size.
